Stomatologické centrum Anděl, s.r.o., with its registered office at Radlická 2000/3, 150 00 Praha 5, Company ID (IČO): 02534720, registered in the Commercial Register maintained by the Municipal Court in Prague under file no. C 220561 (hereinafter the "Company"), will, as the data controller, process your personal data that you enter into the contact form on the Company's website (hereinafter the "Form") and submit to the Company.
Purpose and Manner of Processing Personal Data
Your personal data will be processed for the purpose of responding to your request regarding the Company's products or services as stated in the Form, in particular for the purpose of providing more detailed information about the Company's products or services, offering such products or services, negotiating the conclusion of a contract for the provision of products or services between you and the Company, or carrying out other steps aimed at concluding such a contract. This is therefore the processing of personal data for the purpose of taking steps prior to entering into a contract at your request pursuant to Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter the "Regulation"). Your personal data may further be processed for the purpose of the Company's legitimate interest pursuant to Article 6(1)(f) of the Regulation, in particular for the purpose of protecting the Company's rights in the event of a dispute.
The processing of your personal data is necessary in order to respond to your request stated in the Form and to take further steps prior to entering into a contract between you and the Company, and the provision of personal data is therefore mandatory.
Your personal data will be processed for the above-mentioned purposes until the conclusion of a contract for the provision of products or services between you and the Company. If no contract is concluded, your data will be destroyed within a reasonable period, but no later than six months after the Form is submitted to the Company. If a contract is concluded, you will receive timely detailed information on how your personal data will be processed within the performance of such a contract.
The Company will process the personal data itself. The personal data may further be made accessible to persons who have a lawful reason for accessing the personal data (e.g. supervisory authorities with statutory authorisation to access information), or to other persons where this is necessary for the protection of the Company's rights (e.g. a court).
Your Rights Arising from the Regulation
As a data subject, you have all the rights under the Regulation, in particular the right to obtain confirmation as to whether or not your personal data are being processed, and, where that is the case, the right to access such personal data and other information referred to in Article 15 of the Regulation; the right to request rectification of inaccurate data concerning you pursuant to Article 16 of the Regulation; the right to request erasure of personal data concerning you where one of the grounds set out in Article 17 of the Regulation applies; the right to request that the Company restrict processing where one of the cases referred to in Article 18 of the Regulation occurs; the right to receive the personal data concerning you in a structured, commonly used and machine-readable format and the right to transmit that data to another controller without hindrance from the Company; and the right to lodge a complaint with a supervisory authority if you consider that the processing of your personal data infringes the Regulation; in the Czech Republic, the supervisory authority is the Office for Personal Data Protection, with its registered office at Pplk.
